Waiver and Release for Hinenu Event Participation

There are two steps to attending an in-person event at Hinenu: signing the event waiver and submitting proof of vaccination. Here is the link to submit proof of vaccination: https://hinenu.breezechms.com/form/vaccineupload

Thank you for reviewing and signing this waiver. Everyone attending an in-person event with Hinenu must fill this out prior to attending. We will keep the form on file, you need only fill it out once. A separate form must be submitted for every person over the age of 18.
By participating in this event, I agree to the following community guidelines:

• I will maintain minimum of 6 feet of distance from individuals outside my household/pod for the duration of the event and/or

• I will wear a well-fitting mask that covers my mouth and nose for the duration of the event.

Note: at least one of these mitigation strategies will be required of participants. The specific requirements may vary by venue and will be shared with participants at or prior to the service/event. 

• I do not have symptoms of COVID-19 as detailed by the Centers for Disease Control and Prevention. Specifically, I do not have and am not showing symptoms of any of the following: fever, chills, headache, runny nose, shortness of breath, sore throat, or loss of taste or smell. 

• I have not tested positive for COVID-19 in the past 14 days.

• I have not had exposure to someone who tested positive for COVID-19 in the past 14 days. 

• I did not take a COVID-19 test in the past 14 days because I believed that I had COVID-19 symptoms or was exposed to someone with COVID-19.

• I will alert Hinenu ASAP at rabbi@hinenubaltimore.org if I develop COVID-like symptoms, test positive for COVID, or am otherwise diagnosed with COVID within two weeks after attending a Hinenu event. I understand that this information will be shared with others who attended the event.

I understand that Hinenu is instituting extensive and reasonable safety protocols and precautions to create a safe and healthy environment for all guests. However, despite Hinenu’s best efforts, I understand that Hinenu cannot guarantee that I will not become infected with COVID-19 while attending a service or event hosted by Hinenu.


By attending this event, I hereby release, hold harmless, and waive my right to bring suit against Hinenu, its employees, board members, and/or volunteers due to any potential exposure to COVID-19 in connection with my attendance at a Hinenu event.
